What Is Scrotum Neurotoxin?

Scrotum neurotoxin therapy uses Dysport®, a botulinum toxin type A neuromodulator, to temporarily relax the targeted musculature responsible for scrotal contraction, wrinkling, and sweating. By reducing involuntary contraction of the cremaster and dartos muscles, the treatment allows the scrotum to hang more naturally at rest, smooths the rugae of the scrotal skin, reduces sweating and associated odor, and enhances the overall appearance of the genital area.

The procedure targets two distinct muscle groups through a precise injection protocol. The majority of units are distributed bilaterally along the pathway of the cremaster muscles, which are responsible for scrotal elevation and retraction in response to cold, stimulation, and other triggers. The remaining units are injected into the dartos muscle, targeting the deepest scrotal rugae to smooth the skin surface and reduce sweat gland activity in those areas.

The procedure, its limitations, the temporary nature of the results, and the theoretical fertility considerations discussed below will all be reviewed clearly during this visit.

The Procedure

Treatment is performed in a private, closed-door room. A topical numbing agent is applied prior to injection to ensure comfort throughout the session. Dysport® is then injected using a precise, anatomically guided protocol. Approximately 80 percent of the allocated units are injected bilaterally along the pathway of the cremaster muscle in the groin area, with the remaining 20 percent directed into the dartos muscle at the deepest scrotal rugae. This distribution produces both the lifting and relaxation effect of cremaster release and the surface smoothing and sweat reduction effect of dartos treatment. The procedure is quick, discreet, and performed entirely in-office with minimal disruption to your day.

Duration

Most appointments take approximately 45 minutes from start to finish.

Results

Results develop gradually over 1 to 2 weeks as the neuromodulator takes effect and the targeted muscles begin to relax. Effects typically last 6 months or longer. All effects are fully temporary and reversible as the neurotoxin naturally dissipates.

Who Is Not a Good Candidate?

Scrotum neurotoxin therapy is not appropriate for everyone. Treatment is not recommended for patients who:

• Have a known allergy or hypersensitivity to botulinum toxin or any of its components
• Have an active infection or active skin condition in or around the scrotal area
• Have a bleeding disorder or are taking blood thinning medications without medical clearance
• Have a scrotal condition requiring surgical intervention, such as testicular torsion or other acute scrotal pathology
• Are actively trying to conceive, given the theoretical fertility considerations described below

A Note on Fertility

The dartos muscle plays a role in scrotal thermoregulation, contracting and relaxing to maintain the scrotum at a temperature optimal for sperm health, generally 90 to 95 degrees Fahrenheit. Temporary relaxation of this muscle through neurotoxin may reduce its ability to actively regulate scrotal temperature during the treatment period.

It is important to note that this is a theoretical consideration. Scrotal temperature is influenced by multiple factors beyond the dartos muscle alone, including clothing, ambient temperature, and other physiological mechanisms. There is no clinical study directly demonstrating a meaningful impact on fertility from this treatment. However, out of an abundance of caution, patients who are actively trying to conceive are advised to discuss this with their provider before proceeding. All effects of the neurotoxin are fully reversible as the product dissipates, and any theoretical impact on fertility would resolve completely with the treatment itself.

Aftercare

Following treatment, mild sensitivity or temporary bruising at the injection sites is possible. To support proper healing:

• Avoid Strenuous Activity: limit vigorous physical activity and heavy exercise for the first 24 hours
• Avoid Heat: avoid saunas, hot tubs, and excessive heat for the first 48 to 72 hours
• Avoid Sexual Activity: refrain from sexual intercourse for at least 72 hours following treatment
